  • Quality Improvement Specialist – Full-time (Auburn VNA Health Network)

    Home Healthcare

    Possess sound clinical knowledge and home healthcare practice knowledge. Applicant must have strong problem solving skills. Lead the development and implementation of the Quality Improvement Plan. Conduct QI Committee Meetings in a timely manner. Oversee the patient satisfaction surveys and analyzes results. Report patient satisfaction trends/issues to the Director of Nursing & Therapy and the QI Committee, along with recommendations for improvements. Responsible for OBQI/OBQM reporting and processing. Review all medical record documentation for patient encounters and accurately code services/diagnoses according to ICD-9/10 guidelines. Ensure correct codes assignment based on documentation. This individual must be able to function autonomously, demonstrate excellent problem solving skills, and possess strong clinical and computer competence. Medical Coding and Home health OASIS-C Certificates preferred. *Must have experience in Medicare pre-billing.

  • HR Manager – Full-time (Auburn VNA Health Network)

    Home Healthcare

    The position of HR Manager will report directly to the President/CEO and has oversight of all aspects of the human resource functions. The HR Manager is responsible for coordinating and supervising the development and implementation of plans, policies, procedures and programs relating to Auburn VNA Health Network, Inc. and affiliates. Human Resources will ensure the successful achievement of the Agencies goals and long-term objectives. As counsel to Senior Management, the HR Manager will represent the Human Resource perspective on issues. As a member of senior leadership, the HR Manager is a member of Auburn VNA Health Network's Leadership Team and Committees as designated and will play a vital role in the Agencies Human Resource initiatives.

    Essential experience, skills, and competencies of the Human Resources Manager:

    • 3 to 5 years supervisory experience, required
    • Bachelor's degree or equivalent work experience
    • Minimum of 5-10 years of HR experience
    • PHR or SPHR preferred
    • Healthcare experience, preferred
    • Strong written, verbal, and interpersonal communication skills are essential
    • Proficient use of Microsoft Office Suite

  • Human Resources Coordinator – Full-time and part-time (Auburn VNA Health Network)

    This position provides human resources and clerical support to the Agency through interpreting and administering policies, procedures and assuring compliance with regulatory agencies. Responsible for all aspects regarding HR strategy and leadership, recruitment, recordkeeping, policy development, compensation and benefits, staff development, employee relations, staff tracking and file management, CORI's, COBRA administration and customer service.

    Essential experience, skills, and competencies of the Human Resources Coordinator:

    • Bachelor's degree or equivalent work experience
    • Minimum of 5-10 years of HR experience

  • HOME CARE SCHEDULER (Bancroft Private Care)

    Bancroft Private Care, Inc. a growing private home care agency is in search of an experienced Scheduling Coordinator. Responsibilities include coordination and management of clients receiving private home care services; client screens; field supervisory visits; working closely with clients, families, caregivers, staff and referral sources; answering telephones; and use of scheduling software. Must be a team player, possess excellent problem solving abilities, high attention to detail and display positive interpersonal communication skills. ON-CALL rotation is required.

    ON CALL SCHEDULER

    Bancroft Private Care, Inc. is seeking ON-CALL Scheduling Coordinators to rotate on-call responsibilities for its growing private home care agency. Must have home care scheduling experience, upbeat positive personality, & excellent organizational skills. CHHA preferred.
